Summary: Effectiveness of fibrin clot implantation for reduction of bone tunnel enlargement in bundle-bundle ACL reconstruction using autogenous hamstring tendon grafts


Background; There has been no relevant study quantitatively evaluating the change in tunnel size on CT images in double-bundle ACL reconstruction with autograft. The aim of this study was to evaluate whether adding a fibrin clot to an autograft semitendinosus tendon graft would reduce postoperative tunnel enlargement by means of 3D-CT image analysis. Material and method; sixteen patients who underwent anatomic double-bundle ACL reconstruction with an autograft with fibrin clot and eight patients without fibrin clot were included in the study. The fibrin clot was placed between two strands of the graft end which was inserted to the femoral tunnel. The cross-sectional areas of the anteromedial (AM) and the posterolateral (PL) femoral bone tunnels at 10 mm from the intra-articular tunnel aperture were measured on 3D-CT images at 10 days and 1 year after surgery. Results; The ratios of the femoral tunnel cross-sectional areas between the two time periods were in the graft with fibrin clot group -15.8±24.3% for the AM tunnel and -11.2±24.9% for the PL tunnel ,and in the graft alone group +22.9±20.7% and +24.8±28.6% respectively.
