Topic :- GRAFT AUGMENTED OSTEOSYNTHESIS OF PROXIMAL HUMERUS FRACTURES IN OSTEOPOROTIC PATIENTS

Introduction

:Fractures of the proximal humerus represent approximately 4% of all fractures and 26% of humerus fractures. A locking plate is currently the most widely used method for fixation of these fractures in both young and elderly patients.The use of Locking plate along with intramedullary autograft is an established technique for these fractures in osteoporotic patients.Our study aimed is to assess the functional and radiological outcome of proximal humerus fractures managed with PHILOS PLATE along with graft and followed them prospectively .

PATIENTS AND METHODS :From June 2019 to May 2021 , 20 patients of proximal humerus fracture with DXA SCAN showing osteoporotic bone were operated with PHILOS plate with auto fibular strut graft were followed prospectively for 6 months duration. The outcomes accessed with VISUAL ANALOGUE SCORE (VAS) ,CONSTANT AND MURLEY SCORING SYSTEM, NECK SHAFT ANGLE , HUMERAL HEAD HEIGHT.

Results

:The mean fracture healing is 18wks and average constant and murley score is 90 points out of 100 points. Superficial Infection was found in 2 patients , stiffness in 1 patient and change in NSA was found in 2 patients with minimal change of 1o degree.

Conclusion

:Graft augmented osteosynthesis provides good bone healing and decreases varus collapse and screw lagout with near normal anatomy like normal range NSA angle and good functional outcome and radiological outcome .
