Summary: The responder rate was low in patients with %MA less than 25.9% and FTA greater than 180°, suggesting that alignment correction such as AKO and its combination should be considered.

Background

and Purpose.
To investigate the effect of lower extremity alignment on the efficacy of PRP for KOA.
Methods.
Of the patients registered in 2021 in the registry database of the Japanese Society for Regenerative Medicine (NRMD) at our outpatient PRP clinic, 43 knees of 37 patients (mean age 69.0 years, male/female: 17/20, KL1/2/3/4:2/13/15/13) with complete VAS (>30 before treatment), KOOS, %MA (1-50%) and FTA data before treatment and 6 months after treatment were included in the study. Responder rate was determined by OMERACT-OARSI responder criteria, and ROC curves were generated from %MA and efficacy (SPSS ver. 27).
Results.
The responder rate (RR) at 6 months after PRP was 58.1%, and the mean %MA was 28.3±11.8 (SD). The %MA cutoff for PRP efficacy was 25.9% (sensitivity 76%, specificity 56%, AUC=0.613). The RR for patients with %MA less than 25.9% was 33.3%, and for those with %MA greater than 25.9%, 71.4%. FTA and %MA were correlated (r=0.77, P<0.001), and the predicted FTA was 180.3° when %MA was 25.9%.
Consideration.
The responder rate was low in patients with %MA less than 25.9% and FTA greater than 180°, suggesting that alignment correction such as AKO and its combination should be considered.
