Summary: Comparative study between patients with borderline hip dysplasia and patients with a normal lateral center-edge angle.


Introduction

Hip arthroscopy has become the gold standard treatment for femoral acetabular impingement (FAI) syndrome. The efficacy of this procedure remains controversial in patients with borderline dysplasia. The purpose of this study was to compare functional and clinical outcomes of hip arthroscopy between patients with a Lateral Center Edge Angle (LCEA) of 20 ā€“ 25º and 25 ā€“ 40º.

Method

We conducted a retrospective study assessing 91 female patients that underwent hip arthroscopy in our institution between January 2008 and December 2016. Inclusion criteria were patients over 18 years of age, symptomatic FAI syndrome and 3 years minimum follow-up. Once the patients have been selected, two cohorts were formed; Group A or Borderline Dysplasia (BD), with patients where we observed a LCEA 20 ā€“ 25º and Group B, with patients with a LCEA 25 ā€“ 40º. Functional outcomes were assessed with HHS, HOS and EVA scores. We also registered failure rate.

Results

A total of 91 hip arthroscopies were performed between both groups.
There were 42 patients with Borderline Dysplasia (BD) and 49 with LCEA of 25-40º. Mean age was 32.5 ± 6.6 (range 21 ā€“ 44) years and 34.1 ± 6.5 (range 23 ā€“ 47) respectively, with a mean follow up of 37,1 months (range 36 - 120).
No statistically significant differences was observed comparing both groups regarding HHS and VAS scores (87,0 ± 5,3 versus 85,8 ± 3,6; p=.20 and 1,5 ± 0.6 versus 1.3 ± 0,5 ; p.07)
Assessment of HOS confirmed no statistically significant differences regarding DLA (91.8 ± 6.6 versus 93.2 ± 5.9; p = .28), and Sports (85.1 ± 7.7 ± 8.3 versus 88.3 ± 11.9; p = .19).

Conclusion

Patients with BD undergoing hip arthroscopy achieve good functional and clinical mid-term outcomes comparable with those with a normal LCEA.
