Arthroscopic Lateral Facetectomy for Patellofemoral Osteoarthritis

Summary: In a group of 14 patients we performed Arthroscopic Lateral Facetectomy to treat Iwano II and III osteoarthritis. Arthroscopic Lateral Facetectomy provides clinical improvement in a group of young and active patients with isolated lateral patellofemoral osteoarthritis. This result was better compared with controls treated with physiotherapy.

Abstract:

Isolated Patellofemoral Osteoarthritis has been described in a 34% of women and 19% of men population older than 50 y.o. This problem affect in a 92% lateral facet.
In middle age patients with high level requierements is possible to resect arthroscopically lateral osteophyte and part of the lateral patellar facet to decrease symptoms from this focal wear. Few cases has been reported with this minimal invasive technique.

Method

We included 14 patients with lateral patellofemoral osteoarthritis with a median age of 48 y.o. (43-54) underwent to arthroscopic facetectomy, with at least 1 year follow up and a comparative control group of 14 patients with conservative treatment (physiotherapy). Both groups were grade II and III of Iwano radiologic classification.
Assesment was performed with Kujala Test preoperative, 6 months and 12 months after surgery.
Visual analogue pain scale(VAS) for three activities of daily living ( Rest , climb stairs and stand up ) Preoperative and 1 year follow up.

Results

Clinical results shown a significant improvement in Kujala score in the surgical group from 48,5 to 73 at 6 months and at 72 at 12 months (p>0.05).
In the control group we didn´t find a significant difference between pre and post treatment ( 49,2-50,1)
Median preoperatory VAS was 2(1-4) at rest, 5(3-7 )climb stairs, 5( 3-6 )stand up.
Median postoperatory VAS was 2(0-3) at rest, 1(0-3)climb stairs, 1(0-3)stand up.

Conclusion

Arthroscopic Lateral Facetectomy provides clinical improvement in a group of young and active patients with isolated lateral patellofemoral osteoarthritis.
This result was better compared with controls treated with physiotherapy.
In terms of pain, the relief was higher for pain associated with activity (climb stairs and stand up) compared with pain at rest.
