Arthroscopic Investigation of Cyclops Lesion and Clinical Outcome After Remnant Bundle Preserved Anterior Cruciate Ligament Reconstruction

Summary: Incidence of Cyclops lesions in post-remnant preservation technique can be eliminated by proper patient and technique indication.Proper indication of cases can lead to better outcomes.

Abstract:

Purpose

The aim of this study was to compare the prevalenceand of Cyclops lesions and clinical outcome after anterior cruciate ligamentreconstruction (ACLR) using the different type of remnantbundle and different type of preservation techniques.
Materials and methods:This is retrospective study of 87 knee in which Anterior cruciate ligament reconstruction with every remnant preservation technique was performed by a single surgeon from March 2010 up to December 2012, 9 patient of Cyclops lesion were occurred.2ndlook arthroscopy was performed in each group and pathology was confirmedfocusing on occurringCyclops lesion. The cases were divided two groups as the status of remaining remnant bundle, connective remnant bundle group(n=35) and truncated remnant bundle group(n=52). Also we verified prevalence of Cyclops lesion under thethree different type (simple augmentation, remnant bundle re-tensioning and central passing into the connective remnant tissue) of remnant technique. Under this comparative groups prevalence of Cyclops lesion and clinical outcome was investigated.
Result: There was no difference in the patient characteristics between both groups.All Cyclopslesions were occurred in truncated remnant bundle group(P<0.001).Furthermore, about the technical option, prevalence of Cyclops lesion showed significantly high in the simple augmentation remnant bundle group.(P<0.001). Surgical outcome (Lysholm score, IKDC score) showed no difference between the remaining remnant bundle group and connective remnant bundle group. However, in the groups of remnant technique, re-tensioning of remnant bundle group and central passing among the connective remnant tissue group were slightly improved compare with simple augmentation group.
Conclusion:Connective remnant bundle group showed a low incidence of Cyclopslesion compared with truncated remnant group. And simple augmentation of truncated remnant tissue possibly not be used for preservation.
