Reduced Blood Loss with Low-Volume Topical Tranexamic Acid after RSA

Abstract

Background

Tranexamic acid (TXA) significantly decreases blood loss in hip and knee arthroplasty and has been shown to decrease also blood loss after total shoulder arthroplasty. The purpose of this study was to determine the decrease of blood loss after total shoulder arthroplasty using TXA

Methods

A total of 43 patients who underwent shoulder arthroplasty were prospectively randomized to receive 30 ml of TXA before deltopectoral closure or nothing. All patients received a postoperative drain. Blood loss was considered as drain output, transfusion requirements and change in hemoglobin levels were recorded. All complications were recorded.

Results

The amount of drainage was 155 ml (+/-<DS) in the control group and 72.73 (+/-<DS) in the TXA group (P=0.042). No patient received a transfusion and four complications were observed. One postoperative seroma resolved with ice and compression. The other three were considered not related to the procedure or the intervention.

Discussion

In this cohort of patients, those who received TXA after RSA experienced significantly less blood loss and drain output. The elimination of drainage may be safe in RSA and may reduce postoperative care and associated costs.
