Abstract

Introduction

Clinical presentation of knee osteoarthritis is usually chronic, activity related pain and functional impairment. There are no therapeutic drugs available to halt the disease progression, leaving patients dependent on pain medication, anti-inflammatory drugs, physiotherapy or invasive joint replacement surgery.

Purpose

To assess the safety & efficacy of Autologous Adipose tissue derived Stromal Vascular Fraction using Ultrasonic cavitation in the treatment of Osteoarthritis of knee.

Methods

Patients of osteoarthritis K-L grade 2-4 were selected for the procedure with the care that knee should not have instability & deformity more than 10 degree, patients with neoplasia, infection or any unstable medical condition were excluded. After proper informed consent under all aseptic precaution in tumescent anesthesia liposuction was done to get about 300-500 ml lipoaspirate. This lipoaspirate is then processed and ultrasonic cavitation done and stromal vascular fraction is extracted. Cell viability & counting done and then SVF injection done. Periodic Pre-operative and post operative ( upto 24 month ) KOOS & VAS score were taken. And then results were analysed.

Results

A total of 70 patients and total of 140 joints, with OA knee were treated with SVF. No serious side effects, systemic infection or cancer was associated with SVF cell therapy. All patients improved after the treatment. Average KOOS score improved from 37.5 pre-operatively to 72.6 post-op after 24 months. All sub scale parameter for pain, symptoms, activity of living & quality of life are also improved. Statistical significant improvement noted. A positive correlation was found between BMI of the patients. Also a negative correlation was found between the grade of osteoarthritis and the scores obtained after 6 month of surgery.

Conclusion

we recommend a novel and promising treatment approach for patients with degenerative OA that is safe, cost-effective, and relying only on autologous cells, which can be used as one of the minimal invasive treatment modality for osteoarthritis.
