Arthroscopic Repair Technique of Chronic Irreducible Bucket Handle Meniscus Tears

Abstract

Purpose:To investigate the possibility of arthroscopic suture technique for repairing chronic irreducible bucket-handle tears of the medial meniscus.
METHODS:Between February 2014 and January 2016, 16 patients with Medial meniscus meniscus bucket handle tear underwent arthroscope surgery. There were 7 males and 9 females with 10 left knee and 6 right knee. The symptom include the Knee locking ,Knee pain and Knee extention disfunction. Use of epidural anesthesia,Routine diagnostic arthroscopy to confirm the presence of a bucket-handle meniscal tear. the anterior fragment of the bucket-handle was adherent to the meniscal tear and a "short way" was formed,which makes the meniscal reduction difficult. After removing of the "short way" and freshening of the tear margins, the meniscus were reduced and repaired using FasT-Fix inside-out technique and MM2 outside-in technique. After the operation, the brace treatment and rehabilitation program were routinely used.Clinical results were evaluated preoperatively and at the follow-up of 3,9,12 month using Lysholm IKDC VAS scores and MRI examination.
RESULTS:Based on the MRI examination, meniscus bucket handle tear was successfully repaired in 70.3%. The scores of Lysholm IKDC VAS of the knee at 3 months, 9 months, and 12 months follow-up were significantly better than preoperative ones (P < 0.05).
CONCLUSION:The "short way" repair technique of meniscus bucket handle tear under arthroscopy has an excellent effectiveness, Partial meniscectomy is avoided. It is a convenient and feasible new technology.
