A short-term curative analysis of anterior cruciate ligament reconstruction with remnant preservation

Abstract

Objective

to explore the recent curative effect of patients with anterior cruciate ligament rupture who underwent cruciate ligament(ACL)reconstruction

Methods

From January 2014 to October 2015, a total of 36 patients ( male /32, female /4, age /17 to 42 years old, the average age/ 24.5 years) confirmed anterior cruciate ligament rupture by arthroscopy underwent remnant-preserved anterior cruciate ligament(ACL)reconstruction (retain remnants of 3 cases, keep the end 16 cases, retain residual roots of 17 cases), with positive function recovery after operation , then check the postoperative knee joint range of motion, Lysholm scale, IKDC scores, postoperative review knee MRI, some patients who needed to remove the tibial segment fixation device were taken second Arthroscopic assessment at the same time .[result] 3 patients postoperative recently appeared superficial infection at the tibial medial incision, the incision healing well after dressing change; At the time of the last follow-up: 3 cases of patients with knee range of motion (ROM) limited partly,4 cases of patients still suffered from knee instability and feared to return to the movement; 2 cases with tibia medial paralysis and discomfort; Lysholm score: 63.5±5.7 preoperatively ;Postoperative half year: retain remnants of group 82.1 ±3.4;Retain the residual end group 80.4±5.8;Retain the residual root group 79.6 ±7.1; Postoperative 1 year: retain remnants of group 82.8 ± 2.4;Retain the residual end group 81.2 ± 7.8;Retain the residual root group 80.2± 1.3;IKDC scores: 46.5± 5.9 preoperatively; Postoperative half year: retain remnants of group 66.4± 5.5;Retain the residual end group 65.1 ± 2.3;Retain the residual root group 60.2± 5.8; Postoperative 1 year: retain remnants of group 67.9 ±7.6;Retain the residual end group 66.7±1.6;Retain the residual root group 62.5± 8.9. 24 cases who reviewed Knee MRI after 1 year postoperatively showed the continuity of reconstructive ligament of knee is well ;12 cases who removed the tibia segment door hammer were taken the secondary arthroscopic examination of the knee joint during the same time after 13-16 monthes postoperatively, including 9 cases showed the integrity and tension of ligament were well, 3 cases showed ligamentous laxity, tension were a bit poor.

Conclusion

remnant-preserved anterior cruciate ligament(ACL)reconstruction is an effective way for the treatment of anterior cruciate ligament rupture, but we still need further study to explore If there are more advantages than conventional ligament reconstruction。
