Summary

Free femoral condyle cartilage flap of using drop popliteal artery blood vessels as pedicle repairing huckle-bone osteochondral injury got good early clinical result, and it is a selectivity way of repairing cartilage injury.

Abstract

Purpose

To investigate a new way of repairing and reconstructing osteondral lesions of the talus (Hepple V type).

Methods

The reason of causing osteochondral lesions of the talus is not clearly until now, maybe it is similar with the mechanism of knee joint exfoliative osteochondritis. It may relates with trauma, ischemia and other factors. It is found that free femoral condyle cartilage flap of using drop popliteal artery blood vessels as pedicle can be used to treat navicular bone nonunion by consulting documents[1-3], then making free femoral condyle cartilage flap of using drop popliteal artery blood vessels as pedicle to repair part osteochondral injury will be our main objective.

Results

Free femoral condyle cartilage flap of using drop popliteal artery blood vessels as pedicle is an activity complex tissue flap, and it can repair the bone defect and cartilage injury by transferring the osteochondral complex tissue flap with blood supply to the osteochondral defect part, and it can avoid bone and soft tissue stripping for the reason of bad blood supply, so as to get the first stage biology heal. Our department has done 4 such operations. The AOFAS score of patients are increased to 89 scores of post-operation from 5 scores of pre-operation,VAS scores of patients are improved to 2 score of post-operation from 7 scores of pre-operation.

Conclusion

Free femoral condyle cartilage flap of using drop popliteal artery blood vessels as pedicle can supply enough support and has enough cartilago articularis. It has no obvious influence for donor site, and it is especially suitable to treat osteondral lesions of the talus( Hepple V type). May be it can be a new therapeutic method to treat osteondral lesions of the talus.
