Arthroscopic Fixation of Tibial Avulsion Fractures of the Posterior Cruciate Ligament Using Double Cortical Buttons From Anterior Approach

Summary: Posterior cruciate ligament avulsion fracture of the tibial attachment can be fixed with two cortical buttons from single tibial tunel under arthroscopy with good recent functional results, avoid posterior approach through popliteal fossa, without posterior assistant portals and make the simultaneously arthroscopic examination from the regular anterior portals possible.

From August 2010 to August 2013, 12 patients (7 male and 5 female) with acute posterior cruciate ligament avulsion fracture of the tibial attachment were treated surgically. All of the 12 patients were used double cortical buttons to fix the intact avulsion fragment from anterior approach under arthroscopy without posterior incision of the knee joint. Under the X-ray image intensifier monitoring, one guide wire was drilled from the anterior cortical of the tibial and penetrated the fracture fragment. The single tunnel was made by a 5mm-diameter canulated drill. Induce one of the cortical button through the fragment into the posterior capsule of the knee joint. When the thread between the two cortical buttons was fastened, the fracture was fixed tightly by the turned buttons. The patients were followed for an average of 12.6months; and according to The Inernational Knee Documentation Committee the results were evaluated. The average follow up is 12.6 months(6-30months). All the fractures achieved union at an average of 2.6months. 6 patients with meniscus injury were repaired in the same operation. The function results are good. Arthroscopic double cortical buttons fixation technique can be used in acute posterior cruciate ligament avulsion fracture with good recent result. It can offer sufficient fixation strength to promote bone healing avoid posterior approach through popliteal fossa, without posterior assistant portals and make the simultaneously arthroscopic examination from the regular anterior portals possible.
