Arthroscopic Polyglycolic Acid Sheet Graft for Traumatic Instability of Shoulder Joints with Severely Damaged AIGHL Complex for Regeneration

Summary: For the purpose of the regeneration of the AIGHL complex, we performed the clinical usage of the PGA sheet which was the most suitable material for regeneration of soft tissues of joint experimentally, and obtained the preferable results.

Introduction

Severely damaged anterior band of the inferior glenohumeral ligament complex (AIGHL complex) cases caused high ratio of redislocation and severe limitation of external rotation. We investigated that the polyglycolic acid sheet (PGA sheet) was the most suitable material for regeneration of soft tissues of joint experimentally.

Purpose

We performed the clinical usage of the PGA sheet for the purpose of the regeneration of the AIGHL complex. We evaluated the clinical results of the arthroscopic PGA sheet graft for the regeneration of the AIGHL complex to the cases of the traumatic instability of shoulder joint with the severely damaged AIGHL complex.

Methods

We performed the arthroscopic PGA sheet graft to 35 cases out of 233 cases of the chronic anterior shoulder instability after 2010. Male were 23 cases, female were 12 cases. The average age was 25.6 years old. The AIGHL complex of these cases was absent or severely discrete. We evaluated the clinical results using JSS shoulder instability score and MRI findings. Also we evaluated the limitation of the external rotation and the ratio of the redislocation.

Results

The average JSS shoulder instability score improved from 45.7 points to 91.1 points postoperatively. We did not experience the redislocation cases. There was not any difference of the external rotation comparing with the uninjured side. The MRI findings showed the continuity of the low intensity area from the glenoid to the graft in all cases.

Conclusion

We applied PGA sheet for the purpose of the regeneration of the AIGHL complex. And the clinical results were preferable.
