Early Incidence of Lower Extremity Deep Vein Thromboses After Ankle Arthroscopy: A Retrospective Review of a Large Private Payer Database

Summary: We performed a retrospective review of a large private payer database to determine the incidence of deep vein thromboses during the 90-day post-operative period after ankle arthroscopic surgery. We obtained data from 20222 patients undergoing surgery for sports medicine related conditions and found a 0.33% incidence of DVT.

Background

Ankle arthroscopy is increasingly utilized to treat multiple ankle conditions.
Deep and superficial venous thromboses and pulmonary emboli are rare, yet clinically significant postoperative events that have not been well reported after ankle arthroscopy. Reporting of these endpoints is limited in part by a large number needed to detect. As such we performed a large retrospective database analysis to determine the clinical occurrence of such events.

Methods

We performed a retrospective review of a large private payer medical record database within the PearlDiver server, which is a publicly available Health Insurance Portability and Accountability Act (HIPAA)-compliant national database compiled from a collection of private insurer records. It has more than two billion individual patient records. The information within this database has patient information from 2005 to 2011. Our search was performed using the CPT codes for Osteochondral defect excision, plantar fasciotomy, loose body removal, partial synovectomy, limited and extensive debridement (29891, 29893, 29894, 29895, 29897, 29898, respectively) which are sports medicine procedures done commonly at the ankle and ICD-9 codes 453.40, 453.41 and 453.42 that represent deep vein thrombosis (DVT). Our primary outcome was the three different ICD9 codes for DVT, which include unspecified leg DVT, proximal leg DVT and distal leg DVT. We performed our primary search for patients that had received arthroscopic ankle surgery within a private payer database within the Pearldiver software (Warsaw, Indiana, USA). We extracted the DVT occurrence rate at 90 days to determine the incidence of DVT after arthroscopic ankle surgery.

Results

Our result revealed a total of 20,222 patients that underwent 53,765 procedures with the previously mentioned CPT codes. At the 90 day interval 89 DVTs had developed in 66 patients. This accounts for a 0.33% incidence within the 90 day post-operative period.
