*Active and Associate Members Only. Members are required to have paid all dues owed to ISAKOS. If dues are not paid prior to
registering, they will be requested at the time of registration.
**Allied Health and Resident/Fellow Registrants must provide proof of status
(letter) when picking up badge onsite.
Those who are unable to provide proof of status will be asked to pay the
full registration price.
***Exhibitor representative must provide proof of affiliation (business card)
with the exhibiting company and a photo ID in order to receive their badge.
Badges will not be given to anyone
other than the individual named on the badge.
† Price includes Physiotherapy Course and
Exhibit Access only. Registrants cannot attend ISAKOS
Congress ICLs or other Congress sessions.
Methods of Registration
Attendees may register using one of three methods:
- Online at the ISAKOS website, www.isakos.com/meetings/2011congress
- Mail the Registration form and payment to the ISAKOS Office
- Fax the Registration form and payment to the ISAKOS Office
To pre-register via mail or fax, please
complete the registration form. Make sure to include
all contact information (email address, mailing address, telephone and fax)
to ensure receipt of your Registration Confirmation Email. All
registrations MUST be accompanied by payment. Registrations without
payment will not be processed.
All pre-registered ISAKOS attendees will be
emailed confirmations. If you have not received a confirmation within two
weeks of registering, please contact the ISAKOS Office.
Registering Online
Attendees may register for the 2011 ISAKOS Congress online using their credit card (American Express, MasterCard or Visa). Simply follow the instructions on our secure site to register.
Registering by Mail
Attendees may mail their completed registration form with credit card information or check to the ISAKOS Office. Please note all checks must be in US Dollars from a US Bank. Checks should be payable to ISAKOS.

Payment
Registrants can pay for their registration fees in three ways: by credit card, by check and by wire transfer.
- Credit Card – ISAKOS accepts American Express, MasterCard and Visa. Please note that charges will be
processed immediately when the registration is completed. If your credit card is declined,
please try another card or contact the ISAKOS Office about alternate forms of
payment.
- Checks
– ISAKOS will accept checks for payment of registration fees. The check must be raised by a US bank
in US dollars. The checks must be
mailed to the ISAKOS Office (please do not fax a copy of the check). Please note that your registration
will not be completed if the check cannot be deposited.
- Wire
Transfer – Wire transfers are only available by contacting the ISAKOS
Office. Wire transfers must be
accompanied by a Wire Transfer Authorization form and contain specific
information related to the registrant.
Wire Transfer forms must be faxed to the ISAKOS office with the
registration forms. Please note that
registrations will not be processed until the wire transfer is received in the
ISAKOS bank account.

Cancellation & Refunds
Cancellations will be subject to a US $100 processing fee, and may take up to three weeks to be processed. Requests for refunds must be received in writing in the ISAKOS Office by March 1, 2011. No refunds will be issued after that date. Refunds will not be issued for any events or services included in the Congress that are not used at the discretion of the participant.
On-Site Registration and Badge Pick-up
Attendees seeking to register for the Congress after April 1, 2011 must register on-site in Rio de Janeiro. Badge pick-up will be open on Saturday May 14, 2011 and is tentatively scheduled at the Windsor Barra Hotel.
Attendees will be able to pay for their onsite registrations in US dollars, by credit card (American Express, MasterCard and Visa), or by check drawn in US Dollars on a US bank.
Registration will be open every day at the Riocentro Convention Center during the following hours:

Passport and Visa Requirements*
A valid Passport is required to enter Brazil. Participants from certain countries may be required to obtain a visa to enter Brazil. If you are uncertain of your requirements please consult your nearest Brazilian Embassy/Consulate at your earliest convenience.
If you would like an invitation letter from ISAKOS for visa purposes, you can obtain a PDF invitation letter at no cost or have an original letter on ISAKOS letterhead airmailed to you at the price of US $40. Please visit the Registration Payment Page for Visa invitation letter options.

ISAKOS Pre-Courses
New for the 2011 ISAKOS Congress!
ISAKOS is pleased to announce it will be hosting three pre-courses on Saturday
May 14 including Biologic Treatment
Options for the Knee, Olympic Games: From Basic Science to the Gold Medal
and Spotlight on Top Orthopaedic
Procedures in Sports Medicine.
Cost of the ISAKOS Pre-courses is $100 before January 31, 2011 and $125 after
February 1, 2011. Participation in
multiple Pre-Courses is allowed, however full registration fees must be paid for
each course.
Social Activities
Guest and Spouse Morning Cafe
Enjoy the ISAKOS Congress as a registered guest for the week in Rio de Janeiro. The spouse and guest Morning Cafe includes a daily morning coffee. Registration fees are $40 for the spouse and guest Morning Cafe before January 31, 2011; a great opportunity to meet with new and old friends to enhance your experience in Rio de Janeiro.
Sports Tournament – Tuesday, May 17 14:00
ISAKOS Congress attendees are invited to compete with each other in a football/soccer, tennis and golf tournaments, and will be followed by a Brazilian Barbeque. Registration for the Sports Tournament is required.
Instructional Course Lectures
Instructional Course Lectures are included in the registration fee of the Congress. Please see the Registration Payment page for titles and dates. Space is on a first-come, first-serve basis.
ISAKOS Members
Attendees who are ISAKOS Active and Associate members must be current on all dues payments prior to registering for the Congress. Members will not be allowed to register for the 2011 Congress without payment of their 2011 dues. Any outstanding dues will be requested onsite before the registrant will be granted admission to the Congress. Only Active, Associate and Emeritus membership categories will receive discounted registration rates to the ISAKOS Congress.
Applying for ISAKOS Membership
If attendees wish to become an associate member of ISAKOS to register for the
Congress at the members’ rate, they must submit a completed ISAKOS membership
application prior to registering for the Congress before March 31, 2011.
Applications for ISAKOS membership may be completed online on the ISAKOS website
at
www.isakos.com.
Applications for new ISAKOS
memberships must be received by March 31, 2011 to receive the discounted
Congress registration rate.
The ISAKOS Membership committee will review associate membership statuses for
promotion to active membership status on Thursday, May 19, 2011.
