  3. Can I view the Final Program prior to arriving onsite in Osaka?
    The Final Program is not available prior to the start of the Congress. Please visit the Registration Desk during scheduled hours of operation to pick up your registration packet, including the ISAKOS Congress Final Program.
  4. How can I request a copy of the Preliminary Program?
    If you would like to receive a copy of the Preliminary Program by mail, please email your mailing address to the ISAKOS office at isakos@isakos.com. The preliminary program will be available online in August 2008.
  5. How can I contact the ISAKOS Office?
    • Telephone- The ISAKOS Office is open Monday through Friday from 8:30am-5:00pm, Pacific Standard Time. Due to the high volume of calls received, messages left on the voicemail will be returned as quickly as possible. Please leave all messages in English.
    • Email - Due to the high volume of emails received by the ISAKOS office, emails will be responded to within one week. Please send emails in English – emails in other languages will not be answered.
    • Fax – Please send faxes to the ISAKOS office at any time. Due to the sensitive nature of information on the registration form, please verify that you have the correct fax number of +1.925.807.1199.
    • Mail – Please send all letters, registrations forms, etc. to the ISAKOS office at the following address:

Registration

  1. How much does it cost to register?
    The cost of registration depends on the registration type, how the reservation is completed, and when the registration is done. Please refer to the chart below for information related to registration pricing.

    Online registration opens August 1, 2008.

    Please note that you can save $25 by registering online via the ISAKOS website. Discounts will be taken from the total registration fee automatically before the registration fee is charged. Online registrations must be completed by midnight on March 1, 2009. After March 1, you will need to register onsite in Osaka.

    *Members are required to have paid all dues owed to ISAKOS. If dues are not paid prior to registering, they will be requested at the time of registration.

    ** Allied Health and Resident/Fellow Registrants must provide proof of status when picking up badge onsite. Those who are unable to provide proof of status will be asked to pay the full registration price.

    ***Exhibitor representatives must provide proof of affiliation (business card) with the exhibiting company and a photo ID in order to receive their badge. Badges will not be given to anyone other than the individual named on the badge.
  2. If I am a presenter or member of the faculty, is my registration fee waived?
    Due to the large number of presenters participating in the ISAKOS Congress, presenters’ registration fees are NOT waived. ISAKOS does offer a discounted registration rate for presenters. If you are a presenter who qualifies for a less expensive registration rate (ie – resident/fellow or ISAKOS member), please register at the less expensive registration rate.
  3. I am a sales representative for an exhibiting company, how do I register for the Congress?
    Prior to February 15, 2009, exhibitor representatives can register via the ISAKOS Congress website for a discounted rate of US $600. Exhibitor representatives are unable to register for the ISAKOS Congress until the company they are affiliated with has received an exhibit space assignment and confirmation letter. Exhibitor representatives who do not register prior to February 15, 2009 can register on-site for US $700. Please note: All exhibitor representatives will be required to show proof of affiliation (business card) with the exhibiting company and a photo ID, at the on-site registration desk, in order to receive their badge.
  4. When will the Registration Desk be open onsite?
    The Registration Desk for the 2009 ISAKOS Congress will be open according to the following schedule:

  5. If I am a member of an ISAKOS partner society, do I receive a discount for my registration fees?
    ISAKOS does not offer discounted registration fees to the members of Partner Societies. Registration discounts are only available to ISAKOS members.
  6. How can I pay for my ISAKOS Congress Registration Fees?
    Registrants can pay for their registration fees in three ways: by credit card, by check, and by wire transfer. Please see below for more information:
    • Credit card – ISAKOS accepts Visa, MasterCard and American Express. Please note that charges will be processed immediately when the registration is completed. If your credit card is declined, please try another card or contact the ISAKOS office about alternate forms of payment. Please note that security measures on the ISAKOS website prohibits the processing of multiple transactions on the same credit card. If you would like to charge multiple registrations on the same card, please refer to the information below on processing multiple registrations.
    • Check – ISAKOS will accept checks for payment of registration fees. The check must be raised by a US bank in US dollars. The check must be mailed to the ISAKOS office (please do not fax a copy of the check). Please note that your registration will be revoked if the check cannot be deposited.
    • Wire transfer – Wire transfers are only available by contacting the ISAKOS office. Wire transfers must be accompanied by a Wire Transfer Authorization form and contain specific information related to the registrant. Wire Transfer forms must be faxed to the ISAKOS Office with the registration forms. Please note that registrations will not be processed until the wire transfer is received in the ISAKOS bank account.
  7. I faxed in my registration form, when will I receive confirmation of my registration?
    You will receive your email confirmation one week after the form is received in the ISAKOS Office. If you have not received a response within 10 days, please contact the office as we probably do not have your email address correct. Please note that registration confirmations will ONLY be sent via email.
  8. I would like to change my ICL selections, how do I do this?
    Please visit the online registration system to make any changes to your registration record. No additional payment will be required. Simply make any changes you would like to make, and submit your registration form again. Please note: you will receive a new confirmation email containing the changes/additions to your registration. If you are experiencing difficulties, please contact the ISAKOS office.
  9. I would like to add a registration for the Pre-Course, how do I do this?
    Please visit the online registration system. Additional payment of the pre-course registration fee will be required.
  10. I registered online but haven’t received my confirmation email, am I registered?
    It is probable that your email address was entered incorrectly or your spam filter blocked the sending of the email. Please verify that the email was not placed into your spam folder. Please also visit the ISAKOS website to verify that your email address was entered correctly. To receive another copy of your registration confirmation email, please visit the registration website, search for your record, and request another copy of your receipt.
  11. Does ISAKOS offer one day registrations?
    ISAKOS does NOT offer one day registrations for the 2009 ISAKOS Congress.
  12. I didn’t include a registration for my spouse or accompanying persons when I completed my original registration, can I add them?
    Please visit the online registration system to make any changes to your registration record. Please note: additional payment for the spouse program will be required. Simply make any changes you would like to make, and submit your registration form again. Please note that payment of $40 will be charged for each accompanying person added to the registration. If you are experiencing difficulties, please contact the ISAKOS office.
  13. I am unable to attend the Congress and requested a refund, when and how will my refund be processed?
    Refund requests must be submitted in writing to the ISAKOS Office. Refunds are subject to a $75.00 processing fee, and will be refunded directly to the registrant’s credit card. If the original registration was paid with a check or wire transfer, a refund check will be sent (in US dollars). Refunds will not be sent by wire transfer. Refunds may take up to 30 days to be processed. *Refunds received after February 1, 2009 will not be processed until after May 1, 2009.

Hotel & Travel

  1. Who do I contact to make arrangements for hotel and flight reservations?
    The ISAKOS Office is not involved in the booking of hotel or flight reservations. Hotel reservation information will be available in October 2008 through Kinki Nippon Tourist, the official travel company for the ISAKOS Congress. ISAKOS is not able to assist registrants with the booking of flight reservations.
  2. How do I secure a visa to enter Japan?
    The ISAKOS Office is not involved in securing visas for travel into Japan. A valid Passport is required to enter Japan. Participants from certain countries may be required to obtain a visa to enter Japan. If you are uncertain to your requirement please consult your nearest Japanese Embassy/ Consulate or visit the website of Ministry of Foreign Affairs of Japan.

Presenter Information

  1. If I am a presenter or member of the faculty, is my registration fee waived?
    Due to the large number of presenters participating in the ISAKOS Congress, presenters’ registration fees are NOT waived. ISAKOS does offer a discounted registration rate for presenters. If you are a presenter who qualifies for a less expensive registration rate (ie – resident/fellow or ISAKOS member), please register at the less expensive registration rate.
  2. When will I receive notification if my abstract was accepted for e-poster or podium presentation?
    Notification of acceptance for podium presentations and for e-posters will be emailed to the corresponding author (only) in July 2008.
  3. If I am presenting an e-poster, am I required to provide a physical poster?
    The 2009 ISAKOS Congress will only feature electronic posters. Poster presenters are asked to submit up to 10 Power Point slides that will be available for viewing at computer stations at the Congress, as well as included on a CD that will be distributed to all Congress attendees.
  4. I am unable to present on the date that was assigned to me, is it possible to change my presentation date?
    The ISAKOS Office is unable to make changes to presentation times. If you will be unable to present at your scheduled time, please contact the Office to determine if alternate arrangements can be made.

  1. I have never exhibited at an ISAKOS Congress before; how to I apply for exhibit space?
    Visit the Exhibit page of the ISAKOS Congress website. Under the title Quick Links, select “Exhibit Application” and complete the exhibit application process. When complete you will receive a confirmation email, print a copy of this email and mail it to the ISAKOS Exhibit Manager with two copies of the following:
    • background information of the company
    • product brochures or literature

  2. Does ISAKOS offer Promotional Opportunities to Exhibiting companies?
    ISAKOS offers sponsorship and promotional opportunities exclusively to exhibitors, please see pages 19–21 in the Exhibitor Prospectus for detailed information.

  5. I am a sales representative for an exhibiting company, how do I register for the Congress?

ISAKOS Information: Membership, etc.

  1. If I apply for ISAKOS membership, can I register at the Member’s registration rate? Yes, you can apply for ISAKOS membership and receive the Member’s registration rate. However, please note that new membership applications must be received by January 31, 2009 to receive the Member’s registration rate. Please DO NOT register for the meeting as a non-member if you are applying for ISAKOS Membership – it is better to wait and submit your registration after your membership application has been received and processed. Membership applications may be completed online via the isakos website: www.isakos.com
  2. I am a member of an ISAKOS Committee. Where and when will our committee meetings be held?
    ISAKOS Committee meetings will be held on Saturday, April 4 at the Osaka International Convention Center . Committee members will receive an email regarding the specific time and location of their meetings when the schedule has been finalized. The ISAKOS Office encourages Committee Members to regularly check the Members Only section of the ISAKOS office for scheduling updates.
